Patient advocates are essential to successfully serving our patient population. They serve as a connector between United Family Medicine and underserved individuals and families, as well as community members to promote healthy lifestyles and preventative care. They work with clinic staff to identify uninsured, underinsured and underserved individuals and families in need. They help determine eligibility for health care coverage or a sliding scale fee, and link individuals and families to the necessary services.
Some of the services offered include:
- Assistance with insurance coverage and health plan enrollment
- Referrals to emergency food shelves
- Locating and securing housing
- Child care assistance referral
- Legal referrals
- Translation services — offered at no charge to all patients
