If your income is low, or are uninsured or underinsured, you can receive the quality health care you need at our clinic. We can help you with our sliding fee discount program.
To see if you qualify for this program, we will consider the size of your household and your annual income. We will then determine your discount according to a discount schedule based on federal poverty income guidelines.
A clinic staff member is available to meet with you to see if you are eligible for our sliding fee discount program and to set up a payment plan. To apply for this program, please provide us with copies of your pay stubs for the last month, your W-2 form or your most recent 1040 Income Tax Form.
If you earn too much to qualify for public insurance and cannot afford private insurance, or if you are an elder on a fixed income who struggles with co-pays or your deductible, you may be eligible.
